چکیده

In a previous paper (Nathan, 1959) it was stated that when patients who were apparently completely paralysed by upper motor neurone lesions causing spasticity were treated with concentrated solutions of phenol applied around the spinal roots voluntary movements were sometimes uncovered, and it was stated that this technique was being modified so that it might be used for the treatment of spastic patients who can still walk. Since then considerable experience has been gained with modifications of this technique and from experimental studies

Intraoperative Neurophysiology Monitoring During Selective Dorsal Rhizotomy for Spastic Cerebral Palsy

Selective Dorsal Rhizotomy (SDR) is a neurosurgical procedure currently used as a surgical treatment of children with spasticity in their legs. In SDR, the dorsal roots from L2 to S1 or S2 is partially sectioned. The primary goal of SDR is to reduce spasticity and improve the range of movement with preservation of muscle strength. The dorsal roots involved in spasticity are identified on the ba...

Selective dorsal rhizotomy for children with cerebral palsy: the Oswestry experience.

BACKGROUND Although three randomised control trials have shown that selective dorsal rhizotomy (SDR) reduces spasticity in children with cerebral palsy, a meta-analysis of the results demonstrated that the procedure conferred only small functional benefit on the patient. AIM To determine whether applying strict criteria for patient selection as practised in Oswestry leads to improved outcomes...

Surgical treatment of spasticity by selective posterior rhizotomy: 30 years experience.

BACKGROUND Spasticity is a common neurologic disorder with adverse effects on the patient's function. Conservative management is unsuccessful in a significant proportion of patients and neurosurgical intervention should be considered. The mainstay of surgical treatment of spasticity is selective posterior rhizotomy, i.e., section of sensory nerve roots of the cauda equina. OBJECTIVE To report...

Selective posterior rhizotomy: results of five pilot cases.

We report on two patients with spastic quadriplegia and three patients with spastic diplegia who underwent selective posterior rhizotomy. The mean period of follow-up was 15 months (range, 12-21 months). The patients were assessed preoperatively and at 2 weeks, 3 months, 6 months, and 1 year after surgery. Tests included those for muscle tone (using a modified Ashworth scale), range of passive ...

Dorsal rhizotomy for children with cerebral palsy: support for concepts of motor control.

The results from selective dorsal rhizotomy research suggest that therapists need to question some common clinical assumptions about movement dysfunction. The rationale for performing a selective dorsal rhizotomy is based on the clinical assumptions that spasticity is the underlying cause of disordered movement and that reducing or eliminating the spasticity will improve movement. This article ...
